Applying for student group insurance notice

please bring the following documents:

1. Original copy of the medical treatment receipt (copied document must have a stamped hospital seal).
2. Doctor's certificate of diagnosis. (Hospital seal needed for copied one. Doctor's certificate of diagnosis would need if two hospitals were visited). Remark: diagnosis report is needed for cancer patient applying for the first time.
3. Signature stamp (bring your own signature stamp if you are over 20 years of age) (if under 20 years old; please bring your parent’s signature stamp and ID card.
4. Copies of front and back of the student ID. Temporary student must have a stamp of the school register seal.
5. Copy of the front and back cover of the personal bank account book (must have name and account number) the compensation will deposit to your account.
6. Please paste the copy of the front and back cover of the bank account book and student ID on an A4 paper.

• Remark:
1. Outpatient a service does not included in the coverage, must be hospitalized to be able to apply for compensation claim.
2. Must attach the X-ray for fracture patient.
• please prepared and take all the necessary documents to the Hygiene and Health Care Section at the first floor of the administration building to fill out the application.

Applying for student group insurance notice (death)

Please bring the following documents:

1. Original copy of the medical treatment receipt, stamped hospital seal needed for the copied one. Doctor's certificate of diagnosis would need if two hospitals were visited and hospital seal is needed for every one of the copy.
2. A stamped hospital seal of the death certificated issued by the doctor or a legal medical expert.
3. Copies of both parents’ bank account book, personal identity card, and bring both parents’ signature stamps.
4. Copies of front and back of the student ID. Temporary student must have a stamp of the school register seal.
5. Household register witch include the death of the person and the parents. Applied at the office of affairs of household registration
