Effective time for applying for the claim: must be within the two years after the hospitalization or any unexpected accidents.
Step1:DOCUMENTS NEEDED FOR APPLICATION
- Paper prove from doctor’s diagnosis (copied document must have a hospital seal stamp by the hospital).
- Original copy of the medical treatment receipt detailed list. (Copied document must have a hospital seal stamp by the hospital).
- Claim for fracture must attach with the x-rays.
- A signature stamp. (Parent signature stamp and identity card if under 20 years of age.
- Front and back copy of student identity card
- A copy of the cover of the bank account.
Step2:Get the student group insurance application form from the department of Hygiene And Health Care or download the form from the school web site.
Step3:Fill out the application form for student group insurance claim.
Step4:Turn in the student group insurance application form and all the necessary documents to the department of Hygiene And Health Care.
Step5:Insurance company will sent insurance agent to pick up the claim documents every Tuesday or Hygiene And Health Care section will mail the claim documents to the insurance company.
Step6:COMPENSATION FROM THE INSURANCE COMPANY
Compensation will deposit into your bank account two to three weeks from the day the insurance company received the claim documents.
